Volunteer Application Form
To get started as a volunteer for HHHS, please contact the Coordinator of Volunteer services. A phone number for the Coordinator is located on the right-hand side of this page, or you may complete the Volunteer Sign-Up Form below.
All volunteers will be required to go through a screening process, as described below, and training will be provided. For more information about training, visit our main Volunteer page and scroll down to see the list of possible training opportunities.

Volunteer Screening
All volunteers are carefully screened to determine their effective placement and to ensure the safety of our clientele.
Screening includes:
- An interview with the Volunteer Services Coordinator
- A Police Reference Check (including Vulnerable Sector Screening if required)
- Providing 2 references
- A Driver’s Abstract & Driver Evaluation (if driving)
- A Health Review and TB testing (if volunteering in Hospital or Long-Term Care setting)
